Upon contacting Grata House via phone or on-site tour, the admissions director will explain our treatment program and discuss any questions you may have. The admission team will verify your insurance benefits and clarify the treatment costs.
Admission/Detox
Once admitted to the Program and arrives at Grata House, each client completes an initial intake assessment with a nurse and a biopsychosocial assessment with a clinical social worker. Within 24 hours, our medical director conducts an interview of your medical history and an updated physical that commences the detox process– if a client is beginning to withdraw from a substance.
Throughout the first seven days of detox, a complete treatment plan is created with your primary therapist during further in-depth assessments that look at each individual’s strengths and weaknesses. Upon full assessment, the treatment plan is formed with our clients and their close supports– empowering them with a blueprint of recovery and immediate objectives to achieve while in treatment and long-term goals that carry into their aftercare plan once treatment is completed.
Mid-Treatment
Once refreshed and restored to a level of functioning that reminds you of a time in life when you never felt better—treatment begins to help you rebuild your relationships, financial and career goals, resolving adverse experiences from your childhood and early adulthood and developing the daily coping skills to help you achieve long-term sobriety– once you have come to make that decision on your own and genuinely mean business if only for the first time.

Discharge Planning & Aftercare
Licensed Social Workers/Case Managers work individually with our patients from the first day of treatment to develop an aftercare plan that Grata House clients use. Resources they use while in treatment established back home, wherever that may be. Our case managers Zoom with clients once a week, support them while back at home, assess if further support is needed, and check on how integration back home is going. Clients will have follow-up appointments with providers in their local area, including a primary doctor, psychiatrist, outpatient clinic, Grata step-down clinical services, and therapist within their first week at home.
